I/We hereby declare and authorize that:

1. I nominate Mundy Centre Pty Ltd Trading as Mundy Centre, Registered Tax Agent Number 26121778 as my registered tax agent.

2. I authorize Mundy Centre Pty Ltd Trading as Mundy Centre, Registered Tax Agent Number 26121778 and or its representatives by submission of this form to communicate with any government department including the Australian Taxation Office, financial institutions and any other entities as required in order to assist with the preparation of my tax returns.

3. The information provided to Mundy Centre is true and correct.

4. I understand that the fee for the preparation of the tax return is due as soon as the work is commenced by Mundy Centre regardless of the return being lodged to the ATO.

5. I acknowledge that an invoice will be issued for your services and I agree to pay this invoice in full. I also acknowledge that further action may be taken to secure payment of overdue/outstanding accounts.

6. I authorize Mundy Centre Pty Ltd to deal with ATO on my behalf until otherwise advised.
